69th MLC Supply Requests


This year we are collecting Supply Requests via Airtable rather than Google Forms. The Event Operations team is excited about this new form because it solves the problem of having to request the same items every year. You'll also be able to upload your documents into the form rather than sending me a separate email. The form may take some time to fill out this year, but for future years you will only need to adjust the quantities you requested rather than remember each item.

Filling out the form is different from filling out a Google form, so please read the instructions below before you start entering your request
  1. The form is broken up into 3 sections based on Supply Type: Office Supplies, Tech, and Printing. For each additional item of the same Type but different Quantity, you will need to submit a new form. That means if you want 10 pens, 2 notebooks, and 5 sharpies, you will need to complete the form once to submit a request for pens, a second time for notebooks, and a third time for sharpies. If you want 10 pens, 10 notebooks, and 10 sharpies you can select all three items the first time.
  2. Type in your Program Area
  3. For Office Supplies, click the "+ Add link to a record from the "Non-Tagged/Consumable" table" button. That will open a dropdown menu for all of the items we have in our supply inventory. Click the item you wish to request. Do not click more than one item unless you want the same quantity for all of those items (e.g. one of each for every delegate).
  4. Enter the quantity requested for the item(s).
  5. For Tech, click the "+ Add link to a record from the "Tech Request Summary" table" button. That will open a dropdown menu for all of the items we have in our tech inventory. Do not click more than one item unless you want the samequantity for all of those items.
  6. Enter the quantity requested for the item(s).
  7. For Printing, click the square button with a +paper clip icon to upload a document. Fill out the rest of the questions regarding number of copies, color paper, double sided, preparation, and cutting instructions. Do not upload more than one document unless you want the same quantity and preparation for all documents.
  8. If you need additional items, click the "Submit Another Response" button. Repeat steps 2-8.
