Health History & Authorization Forms

All of the California YMCA’s programs require that everyone in attendance submit a copy of a completed Health History/Authorization form. This includes advisors, ambassadors, delegates, interns, secretariat and volunteer staff. If you participate in multiple programs (ML/C, MUN, Spring Conference, etc…) during a given program year, you need to submit a new form each time.
